Crossroads Community Ministries, a non-profit provider of supportive services for individuals and families experiencing or at-risk of homelessness located in Atlanta, GA, is seeking qualified applicants for the position of Case Manager (Full-time and on-site, 35-hour work week, Monday through Friday, 9am-4pm) in our Renewal Project program. The Renewal Project is a 180-day stabilization program that offers essential services that help individuals progress toward ending their homelessness. Core services include individual counseling and assessment, case management, a mailing address and mailbox access, documentation services, and MARTA passes for employment and medical emergencies. Applicants must have at minimum 3 years’ experience in providing case management (preferably for those experiencing homelessness). A degree in a related field is desired but not required (BSW or MSW preferred). Certification in ClientTrack HMIS required. Applicants must have their own vehicle in good working condition and insured as travel to mobile sites to provide services will be required. Applicants must also be willing to use their personal cell phone in the performance of job duties.
The Case Manager will be responsible for facilitating documentation services and partnering with community resources to address the documentation needs of an assigned case load. This position will report directly to The Renewal Project Program Manager. General case management responsibilities include, but are not limited to, client screening, intake, assessment, service plan development, monitoring, linkage to appropriate community resources, follow up, appropriate discharge and tracking.
Case Management Guidelines:
- Provide on-going assessment and support.
- Maintain thorough and complete record of client’s activity/progress.
- Accurately identify and prioritize at risk client population.
- Document results of above components in HMIS.
- Demonstrate knowledge and skill necessary to provide service appropriate to the age/maturity/mental capacity of the client.
- Change practice behavior as a result of data by client.
- Prepare and submit reports to supervisor as requested.
- Handle difficult situations in a discreet manner.
- Set goals and time frames for achievement of goals. Also set goals for transition to permanent or support housing.
The Case Manager will primarily be responsible for assisting clients in securing birth certificates and state issued ID's. Responsibilities for documentation assistance include, but are not limited to:
- Helping individuals experiencing homelessness with obtaining vital records (birth certificate and state identification) via mobile site services and at the CCM site.
- Creating and sustaining relationships with partner organizations within the area.
- Inputting orientations into ClientTrack after providing services.
- Documenting services provided into ClientTrack, documenting when the documents have been received by Crossroads and when they were returned to the guest.
- Tracking the status of birth certificates for guest ordering birth certificates from out of state.
- Educating guests on what is needed to obtain their state ID prior to them visiting DDS.
- Creating a schedule that flows and meets the needs of the clients in our community. This includes meeting guest at CCM site or visiting centers to assist individuals with birth certificate needs.
- Taking advantage of various trainings that will educate case managers on how to better communicate and serve those experiencing homelessness.
- Collaborating with Crossroads team members to serve our guests and provide a lending hand when needed.
- Providing in-office assistance when I am not scheduled to provide mobile documentation services.
- Identifying resources (attorneys and other avenues) to apply for birth certificates for individuals experiencing homelessness.
- Educating yourself on each state's protocol for applying for birth certificates. This enables Crossroads to apply for more birth certificates on behalf of our guests.
- Picking up GA Birth Certificates.
- Being kind, empathetic and understanding to those you serve. It is a very tough journey, and the documentation specialist must realize they are not here to just provide a service. Our services sometimes include providing a listening ear or message of hope.
